Description

Offered by the Delta Dental Foundation (DDF) with financial support from Delta Dental of Michigan, Ohio, and Indiana.

This program consists of two separate scholarship award categories: 

  • Supply Kit Award. NEW Dental hygiene students about to enter an accredited hygiene program may apply for up to $5,000 to cover the cost of instrument kits and other necessary equipment.
  • Tuition Award. Students currently accepted to a dental hygiene program in Michigan, Ohio or Indiana can apply for scholarships, from $1,000 to $10,000 per year. Students can apply for scholarships each year for up to two years. 

Students can apply to both programs within this application if eligible. Priority will be given to individuals with a financial need. If selected, the tuition funds will be paid directly to the school. 

To be eligible, applicants must: 

  • Be accepted into an accredited dental hygiene program in Michigan, Indiana or Ohio. A letter of consideration from an accredited dental hygiene program is also acceptable. If accepted into the program, a formal letter of acceptance will be required before funds will be disbursed.
  • Submit a completed application and provide all associated documentation for review during the open applicable period.
  • Participate in a scheduled interview(s) with the Selection Committee.

Individuals not eligible to participate in the program include:

  • Individuals who have defaulted on federal loans.
  • Immediate family members of employees and board members of Delta Dental Plan of Michigan, Inc., Delta Dental Plan of Ohio, Inc., Delta Dental Plan of Indiana, Inc., and the Delta Dental Foundation.

Continued Program Eligibility
Recipients who are chosen to participate in the program will need to verify compliance with the program requirements on at least an annual basis (or more frequently as may be necessary to ensure compliance).  Such verification may include:

  • Proof of good academic standing in their dental hygiene program.
  • Participation in scheduled interviews with the DDF to discuss your experience with the program.
  • Provide annual updated Expected Family Contribution as defined by FAFSA for the current academic year.

A recipient’s failure to maintain ongoing compliance with program requirements may result in termination from the program and a loss of scholarship funding.
